A lead comes in through a form. It lands in the CRM. It gets assigned to a rep. Every step of that looks like ownership is clear. In practice, it rarely is. The form assumes the CRM will pick it up. The CRM assumes the rep will act on it. The rep assumes someone will flag it if it's urgent. Everyone's technically responsible, which usually means nobody actually is.

Make ownership explicit at every handoff
I've seen this exact gap stall real leads — not because anyone dropped the ball on purpose, but because the system never forced a single, clear owner and a single, clear next action. It just moved the lead from stage to stage and called that progress.
The fix isn't more people checking in. It's making ownership explicit at every handoff — one person, one next action, always visible, never assumed. If a lead sits without a defined next step and a defined owner, that's not a person failing. That's the system never actually assigning the job in the first place.
